The advent of direct oral anticoagulants (DOACs) has provided alternative oral anticoagulation options for the prevention of stroke and systemic embolism (SSE) secondary to non-valvular atrial fibrillation (NVAF)

An anticoagulant review clinic pilot was developed in south-east Essex, to target patients receiving inadequate anticoagulation with a vitamin K antagonist. The service provided dedicated consultation time with an anticoagulation pharmacist to reach a joint, informed decision regarding the best anticoagulant for the patient and was delivered using existing infrastructure from the anticoagulant monitoring service.
