The winners of the Pharmaceutical Care Award 2015 discuss their primary care project.

A team from Coventry and Rugby Clinical Commissioning Group was judged the winner of the Pharmaceutical Care Awards 2015. Medicines management pharmacists Rosalyn Payne and Sharon Coane describe their winning project — a pharmacist-led initiative that helped reduce polypharmacy in patients aged 80 years and older.
