Pharmacy tsar calls for examples of good practice
A call has been put out to gather examples of good practice that have enabled effective communication between hospital and community pharmacy.
Martin Stephens, national clinical director for hospital pharmacy, said in a statement: "To support the forthcoming changes to the community pharmacy contractual framework it has been agreed that a national template for referral from hospital to community pharmacy would be helpful. This would foster engagement and help avoid duplicated effort. We are aware that a number of localities will already be looking at this issue and may have implemented a means of doing this in practice. We would be keen to see any examples of referral forms or good practice that enables communication between hospital and community pharmacy."
