The new NHS Patient Safety Strategy has a big hole in it

A succinct point about 'wicked' risks by Mr Cousins. Such risks need to be communicated in a clear and defined manner to all medicine and pharmacy sectors. Unfortunately, I cannot glean much information from the NHS single structured sentence response here as a medication safety program should already be in place. I'm interested to know how patient safety officers will help in the process. Medicine safety should be a well defined and simple process to reduce inertia in getting the information out to clinicians.
